Policies
By booking an appointment, you agree to pay for services rendered regardless of insurance coverage. We will bill your insurance for covered services, and you are responsible for any copays, deductibles, and non-covered services. A valid card on file is required, and any patient balances will be charged automatically.
Important Requirements:
All pre-appointment paperwork must be completed 24 hours prior to the day of your appointment before seeing your provider. Failure to complete paperwork will result in cancellation at the patient’s expense.
Physical assessments are required at least every three months for patients prescribed controlled substances.
Appointment Information
Before Your Initial Appointment
We recommend contacting your insurance company to understand your potential financial responsibilities under your behavioral health benefits.
Confirm that TTK Psychiatry Matters is in-network with your plan.
Appointment Fees
All fees are due in full at the time of service.
Initial Diagnostic Evaluation; 60 min; $375
Follow-up (Medication Management/Refill Session); 15 min; $150
Follow-up (Medication Management + Talk/Supportive Therapy Refill); 30 min; $300
Additional Fees (may not be covered by insurance):
Paperwork: $75 per five pages
Physical Assessment: $30 per session
Legal Services: $500 per hour
No-Show & Cancellation Policy:
Missed appointments will be charged the equivalent of the scheduled visit (not billed through insurance).
After two missed appointments, the patient may be discharged from the practice.
Cancellations made less than 24 hours before the appointment will be charged the full appointment fee.
